They're different classes of devices. A modem is comparatively dumb and its sole job is to convert signals from one electrical format to another, typically one able to be sent down a phone line. Anything sent to the modem is converted and sent down the connection regardless of what it is. The conversion only extends to electrical formats - the actual data is sent unchanged.
A router is more intelligent and looks at the data first to decide where to send it - this may be down a phone line or out back another local network connection. Unlike a modem link, which is logically equivalent to simple peice of wire (the conversion to and from different formats is invisible to the devices at either end) a router connects entire networks together and uses its knowledge of network protocols to decide how to process the data. A modem requires another device (typically a computer or a router) to make the decision to send the data down the line and convert the data into a form that logically makes sense at the other end. A router makes this decision and performs the necessary processing of the data by itself.
As for telling the difference that is difficult in the general case and it may not be immediately clear which a particular device is even to a network professional - statements such as countign the number of ports or the like are not reliable indicators. However, for the kind of devices typically used in home installations if it has a WAN port (for the phone or cable line) and more than one ethernet port that is almost certainly a router. If there is no ethernet at all and only a serial or USB connection to a computer that is almost certainly a modem. If there is a single ethernet connection that could be either and you would need to investigate in much greater depth to determine which it is.
